MY FELLOW CLAY COUNTIANS ... & surrounding neighbors ...
Those of you who are located in Clay, Boone, Fayette, Putnam & Kanawha counties in WV, please, please, PLEASE take a few minutes to complete this survey. We can't fill needs if we don't know what they are! Thankyouverymuch!
EnAct, Inc. is conducting a community needs assessment in order to determine causes of poverty in Boone, Clay, Fayette, Kanawha and Putnam Counties and mobilize resources in a plan to meet community needs. EnAct would like to have as many people as possible in these counties to take the brief survey.
The process involves gathering feedback from area residents by asking survey questions about key poverty indicators including education, employment, housing, health, nutrition, use of income, transportation, emergency situations and children’s needs. They will use this information to guide the organization, strengthen services and ultimately have a positive impact in our community. Providing feedback is easy as logging onto:
The survey takes approximately 5-10 minutes and must be completed in one sitting. Deadline for survey completion is July 31, 2014. There are no right or wrong answers, and feedback from all is greatly appreciated!
